  • Enter the full name of the client/patient having the appointment.
  • Choose the preferred call type (Facetime iPhone, Signal App Android, Landline/Regular Call).
  • Indicate the province you live in.
  • Confirm whether your need for care lies within urgency/rural underserviced areas and no other care options are available.

Appointment Confirmation
Upon completion, an appointment confirmation will be sent to you.
